We are seeking a highly skilled and compassionate Community Support Team Lead to join our team at True Team Medical. As a key member of our community-based therapy and case management team, you will provide essential support and services to adults with serious and persistent mental health issues.
Key Responsibilities:
- Conduct assessments and provide community-based therapy and case management to adults.
- Connect clients with higher levels of care, such as ACT services, as needed.
- Assess and reassess client needs for services, informing them about benefits, community resources, and services.
- Assist clients in accessing benefits and services, arranging for them to receive benefits and services, and monitoring the provision of services.
- Provide assistance and support to mental health consumers in crisis situations, as well as service coordination activities, psycho-educational and support activities, and rehabilitative and independent living skills.
Requirements:
- At least one year of experience working with the population served.
- Provisional or Full license in NC (LCSW/A, LCMHC/A).
