Seeking a dedicated Hospice Nurse for a leading healthcare organization.

Compensation:
$38 - $43 per hour

About the Role:
We are looking for a committed and empathetic Permanent RN Liaison/Navigator to enhance our healthcare team.

The ideal candidate will be instrumental in ensuring a smooth transition of care for patients moving from hospital settings to home, nursing facilities, or hospice care.

You will collaborate closely with patients, their families, and various healthcare professionals to guarantee timely and appropriate care delivery.

This position demands a comprehensive understanding of clinical practices, excellent communication abilities, and the capacity to operate independently while being part of a cohesive team.


Key Responsibilities:

  • Act as the main contact for patients and their families, offering guidance and education throughout their care journey.
  • Work alongside healthcare professionals to create and execute personalized care plans, ensuring services align with patients' specific needs and preferences.
  • Oversee and facilitate the transition of patients from hospitals to home, nursing facilities, or hospice, ensuring a fluid process.
  • Advocate for patients and families, ensuring their concerns are addressed and their needs are fulfilled.
  • Track patient progress, modifying care plans as necessary and communicating updates to all involved parties.
  • Provide emotional support and counseling to patients and families, assisting them in navigating the complexities of the healthcare landscape.
  • Engage in continuous education and training, remaining informed about the latest best practices in patient care and navigation.

Qualifications:

  • Valid and unrestricted Registered Nurse (RN) license.
  • Minimum of 2 years of experience in a clinical environment, ideally in a liaison or navigator capacity.
  • Preferred: Certified Hospice and Palliative Nurse (CHPN) certification.
  • Required: Basic Life Support (BLS) certification.
  • Outstanding communication and interpersonal skills, with a talent for building strong relationships with patients, families, and healthcare providers.
  • Robust clinical knowledge, particularly in hospice and palliative care.
  • Ability to work autonomously and make critical decisions while also functioning as part of a larger team.
  • Familiarity with electronic medical records and other healthcare technologies.
  • Compassionate and patient-centered, with a strong dedication to delivering the highest quality of care.
