About Us:
We are in search of a dedicated, empathetic, and patient-centered Permanent RN Liaison/Navigator to enhance our exceptional healthcare team.

Job Requirements:

Qualifications:

  • Current and valid Registered Nurse (RN) license.
  • Minimum of 2 years of experience in a clinical environment, ideally in a liaison or navigator capacity.
  • Certification as a Certified Hospice and Palliative Nurse (CHPN) is preferred.
  • Basic Life Support (BLS) certification is mandatory.
  • Outstanding communication and interpersonal skills, with the ability to cultivate strong relationships with patients, families, and healthcare providers.
  • Robust clinical knowledge, particularly in hospice and palliative care.
  • Capability to work independently and make critical decisions while also functioning as part of a cohesive team.
  • Familiarity with electronic medical records and other healthcare technologies.
  • Compassionate and patient-oriented, with a strong dedication to delivering the highest standard of care.
