About Us:
We are looking for a dedicated and empathetic Permanent RN Liaison/Navigator to enhance our healthcare team.

The ideal candidate will be instrumental in ensuring a smooth transition of care for patients moving from hospital settings to home, nursing facilities, or hospice care.

Your role will involve close collaboration with patients, families, and healthcare professionals to guarantee timely and appropriate care delivery.

This position demands a solid grasp of clinical practices, excellent communication abilities, and the capacity to operate independently while being part of a cohesive team.


What You Will Do:

  • Act as the main contact for patients and their families, offering guidance and support throughout their healthcare journey.
  • Work alongside healthcare professionals to create and execute personalized care plans that cater to patients' specific needs and preferences.
  • Oversee and facilitate the transition of patients from hospitals to home, nursing facilities, or hospice, ensuring a seamless process.
  • Champion the needs of patients and families, ensuring their concerns are addressed and their requirements are fulfilled.
  • Track patient progress, modify care plans as necessary, and communicate updates to all involved parties.
  • Provide emotional support and counseling to patients and families, assisting them in navigating the complexities of the healthcare landscape.
  • Engage in continuous education and training to remain informed about the latest best practices in patient care and navigation.

Position Requirements:

  • Valid and unrestricted Registered Nurse (RN) license.
  • Minimum of 2 years of experience in a clinical environment, ideally in a liaison or navigator capacity.
  • Preferred: Certified Hospice and Palliative Nurse (CHPN) certification.
  • Required: Basic Life Support (BLS) certification.
  • Outstanding communication and interpersonal skills, with the ability to forge strong connections with patients, families, and healthcare providers.
  • Robust clinical knowledge, particularly in hospice and palliative care.
  • Ability to work autonomously and make critical decisions while also collaborating within a larger team.
  • Familiarity with electronic medical records and other healthcare technologies.
  • Compassionate and patient-centered, committed to delivering the highest standard of care.
