Job Opportunity

Professional Management Enterprises is seeking a highly motivated and organized individual to join our team as a Community Outreach Coordinator. This role will be a member of the Health, Education and Access Team (HEAT) and will represent UnitedHealthcare Community Plan while educating and informing enrolled members on various topics related to health, well-being, and navigation of the healthcare system.

Key Responsibilities:

  • Represent UnitedHealthcare Community Plan by promoting its core values and services
  • Conduct new member health needs screenings, thoroughly documenting interactions with members and their support networks
  • Educate and inform people of diverse backgrounds and cultures
  • Collaborate across the team and with health plan leadership to share barriers and best practices that impact the lives of our members

Requirements:

  • Two or more years of education and/or experience in a customer-facing role
  • Ability to effectively work in a 100% remote/work-at-home environment
  • Willingness to work a flexible 40-hour week, including occasional evenings and weekends
  • Excellent organizational skills
  • Self-directed and independent in completing tasks
  • Professional communication skills in both verbal and written form
  • Attention to detail and accuracy

Preferred Qualifications:

  • Community Health Worker certification
  • Advanced skill set working with Excel (sorting, filtering, pivot tables)
  • Experience working with diverse populations, including individuals who are aged, blind, or disabled, and health professionals and providers
  • Bilingual in Spanish, Burmese, Arabic, or French Creole
