Job Overview

BCForward is seeking a skilled Care Coordinator II to support care management activities and ensure services are delivered by healthcare providers and partners.

The ideal candidate will have a strong background in OB and experience in providers' offices or hospital settings. They will be responsible for coordinating care activities, interacting with members, and documenting care plans.

This is a remote opportunity with a pay range of $21.00/hr on W2. The expected duration is 8 months of contract to hire.

Key Responsibilities
  • Supports care management activities and ensures services are delivered by healthcare providers and partners.
  • Coordinates care activities based on the care plan and works with healthcare and community providers and partners.
  • Interacts with members by performing member outreach telephonically or through home-visits and documents the plan for care/services of activities.
  • Provides education on benefits and resources available to members.
  • Performs other duties as assigned.
Requirements
  • High School diploma or GED.
  • 1-2 years of related experience.
  • Organized and able to handle a fast-paced environment.
  • Knowledge and comfortable navigating Microsoft software (Word, Excel, Teams).
