We are seeking a highly skilled and compassionate Physical Therapist to join our team in providing community-based outpatient services to homes in the Newark, NJ area.
Job Summary:The successful candidate will work with a multidisciplinary therapy team to provide evidence-based care to a well-balanced mix of patients in their homes. This is a full-time, part-time, or PRN position that includes a flexible schedule/hours per week based on your preference.
Responsibilities:- Provide community-based outpatient services to homes in the Newark, NJ area.
- Work with a multidisciplinary therapy team to provide evidence-based care to patients in their homes.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Conduct thorough assessments and evaluations to identify patient needs and goals.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduation as a Doctor of Physical Therapy (or equivalent standing).
- Eligibility for New Jersey state licensure as a Physical Therapist.
- Availability to work a minimum of 12 hours per week of direct patient care.
- Regular and consistent weekly availability is required, which includes 3 weekdays of availability.
- Professionalism, accuracy, and dependability are essential.
