LPN Home Health Care Specialist

We are seeking a skilled and compassionate LPN to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality nursing services to patients in their homes, promoting independence, dignity, and comfort.
Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans, focusing on patient-centered care and promoting optimal health outcomes.
- Conduct thorough assessments, identify patient needs, and develop effective interventions to address those needs.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and healthcare teams, ensuring clear understanding of care plans and progress.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
