Job Title: LPN Nurse - Skilled Care

Job Summary: We are seeking a qualified Licensed Practical Nurse to join our team of professionals at Evergreen House Health Center.

Responsibilities:

* Deliver quality nursing care to patients through interpersonal contact
* Provide care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being
* Monitor and report changes in the patient's condition
* Assign duties, instruct, and provide clinical supervision to CNAs

Requirements:

* Graduated from an accredited LPN program
* Currently licensed/registered in applicable State
* One (1) year skilled nursing experience preferred
* CPR certification upon hire or obtain during orientation
