Job Summary:
We are seeking a skilled and compassionate Certified Home Health LPN to join our team at connectRN Inc. As a Certified Home Health LPN, you will provide high-quality patient care and support to patients in their homes, promoting independence and improving health outcomes.
Responsibilities:
- Provide one-on-one skilled nursing care and treatment to patients in their homes
- Assist the registered nurse in assessment of patient's condition and contribute information to aid in planning and evaluating care
- Perform comprehensive patient assessments and collaborate with a multidisciplinary care team to develop an individualized plan of care as per physician orders that achieve desired outcomes and goals
- Check patient's vital signs, perform wound care, and educate patients and their families about their medications and chronic conditions
- Provide patient episode management per protocol
- Complete documentation timely and accurately according to industry standards, conventions, and guidelines, including subsequent visits, or other required documentation
Requirements:
- Current and unencumbered license as a Licensed Practical Nurse (LPN)
- Strong assessment, critical thinking, and communication skills
- 1+ year(s) of Certified Home Health experience as an LPN
- Ability to work independently and as part of a dynamic team
- Comfortability using technical tools and software/EMR for visit documentation
- Current CPR certification
- Transportation, current driver's license, and car insurance coverage
- Proof of Immunization/Exemption for Flu and COVID-19
- TB Test or Chest X-Ray
