Responsibilities:

  • Provide one-on-one skilled nursing care and treatment to patients in their homes
  • Assist the registered nurse in assessment of patient's condition and contribute information to aid in planning and evaluating care
  • Perform comprehensive patient assessments and collaborate with a multidisciplinary care team to develop an individualized plan of care as per physician orders that achieve desired outcomes and goals
  • Check patient's vital signs, perform wound care, and educate patients and their families about their medications and chronic conditions
  • Provide patient episode management per protocol
  • Complete documentation timely and accurately according to industry standards, conventions, and guidelines, including subsequent visits, or other required documentation

Requirements:

  • Current and unencumbered license as a Licensed Practical Nurse (LPN)
  • Strong assessment, critical thinking, and communication skills
  • 1+ year(s) of Certified Home Health experience as an LPN
  • Ability to work independently and as part of a dynamic team
  • Comfortability using technical tools and software/EMR for visit documentation
  • Current CPR certification
  • Transportation, current driver's license, and car insurance coverage
  • Proof of Immunization/Exemption for Flu and COVID-19
  • TB Test or Chest X-Ray
