Care Coordination Opportunity

Metropolitan Family Services is seeking an experienced Care Coordinator to join our team in Chicago. This is a full-time position that requires a strong understanding of care coordination principles and practices.

The estimated salary range for this position is $47,000 - $52,000 annually, depending on qualifications and experience.

Key Responsibilities:

  • Link clients to necessary community resources and services.
  • Communicate effectively with clients, families, and healthcare providers.
  • Document client interactions and progress in electronic health records.
  • Participate in multidisciplinary teams to develop comprehensive care plans.

Requirements:

  • Bachelor's degree in Social Work or Human Services related field.
  • 1+ year experience in care coordination or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment.
