Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Field Nurse. As a Field Nurse, you will provide high-quality patient care in the comfort of their own homes, utilizing the nursing process to assess, plan, implement, and evaluate patient care.

Key Responsibilities
  • Assess the physical, psychological, social, and spiritual needs of patients and families within 24 hours of admission.
  • Develop and implement individualized plans of care, including measurable patient-centered goals, in collaboration with physicians and patients.
  • Review and update plans of care to reflect changing patient needs, including interventions, medication profiles, and visit frequencies.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and support to patients and families on disease management, medication adherence, and self-care.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care, including hospice care, palliative care, and other supportive services.
  • Document patient care and outcomes accurately and thoroughly in the electronic health record.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) degree.
  • Active Registered Nurse license to practice in the state of Florida.
  • 1-2 years of nursing experience, preferably in hospice or palliative care.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
