Job Title: Per-diem Inpatient Hospice Nurse

We are seeking a compassionate and skilled Per-diem Inpatient Hospice Nurse to join our team at Hope Hospice. As a trusted community resource since 1980, we pride ourselves on providing quality care to patients and families in need.

About the Role:

This per-diem position offers competitive pay, generous benefits, and a supportive work environment. As an Inpatient Hospice Nurse, you will provide nursing care to patients who require GIP level of care for end-of-life symptom management and pain control in collaboration with the Stanford Hospital inpatient team.

Responsibilities:
  • Perform initial and ongoing hospice eligibility assessment and consultation with Hospice MD in collaboration with Stanford and in accordance with CMS guidelines.
  • Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement, and evaluate patient/family needs, goals, and interventions.
  • Perform evaluation and coordination of social work and spiritual support services needed by patient/family.
  • Coordinate the implementation of the plan of care and ensure care coordination with the inpatient team.
Requirements:
  • One year experience as a professional nurse in the past three years or; Bachelor's degree in Nursing from an accredited School of Nursing.
  • Experience in acute care/home health and/or hospice setting preferred.
  • Current license as a registered professional nurse in the State of California.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.
  • Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car.
