About the Role

Bristol Hospice is seeking a skilled Clinical Liaison LVN to join our team. As a key member of our community outreach program, you will play a vital role in building relationships with healthcare professionals, patients, and families to promote our hospice services.

Key Responsibilities
  • Develop and maintain relationships with referral sources, including assisted living facilities, hospitals, and physician offices.
  • Ensure patients and their families receive accurate information about hospice care and services.
  • Collaborate with community partners to promote our services and programs.
  • Participate in regular team calls to discuss patient care and community outreach initiatives.
  • Drive growth and expansion of Bristol Hospice through advocacy and education.
Requirements
  • Graduate of an accredited school of nursing with a current LVN/LPN license.
  • Minimum one year of experience as an LVN/LPN within the last three years.
  • CPR certification and valid driver's license.
  • Excellent communication and interpersonal skills.
  • Ability to articulate the Hospice Conditions of Participation and facilitate transitions in care.
  • Minimum one year of sales, marketing, professional relations, and/or education training.
  • Flexible scheduling, including evenings and weekends, to meet patient and family needs.
