About the Role

We are seeking a compassionate and experienced Home Health Aide to join our team at Bristol Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Provide personal care services to patients, including bathing, dressing, and grooming, as assigned.
  • Assist patients with daily living activities, such as feeding, toileting, and transferring.
  • Monitor and report any changes in patient condition to the Case Manager.
  • Assist with light housekeeping and laundry duties as needed.
  • Participate in quality assessment and performance improvement activities.
Requirements
  • Certification as a Nursing Assistant or Home Health Aide, as required by state regulations.
  • One year of experience as a Home Health Aide in a community health or hospice setting, or medical/surgical experience.
  • Completion of a minimum of 75 hours of classroom and supervised practical training.
  • Licensed driver with a vehicle that is insured and in good working order.
  • Ability to pass a criminal background check and maintain current CPR certification.
Preferred Qualifications
  • One year of experience as a Home Health Aide in a community health or hospice setting.
  • Ability to work autonomously with minimal direct supervision.
  • Strong organizational and communication skills.
  • Ability to empathize with the needs of the ill, injured, frail, and impaired.
