We are seeking a compassionate and experienced Certified Nursing Assistant (CNA) and Home Health Aide (HHA) to join our team at Bristol Hospice. As a member of our Interdisciplinary Group Team, you will play a critical role in providing high-quality care to our patients and their families.
Key Responsibilities- Provide personal care services to patients, including bathing, dressing, and grooming, as assigned.
- Assist patients with daily living activities, such as feeding, toileting, and ambulation, as directed.
- Monitor patients' conditions and report any changes or concerns to the Case Manager.
- Participate in quality assessment and performance improvement activities to ensure the highest level of care.
- Adhere to the organization's documentation and care procedures, as well as standards of personal and professional conduct.
- Certification of Nursing Assistant or Home Health Aide required per state regulations.
- Preferred to have one (1) year experience as Aide in community health/hospice or medical/surgical experience.
- Must have completed a minimum of 75 hours of classroom and supervised practical training.
- Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order.
- Willingness to submit to a criminal background check prior to hire and annually thereafter.
- Current CPR certification.
