Candidates should possess a Bachelor's degree in Psychology, Social Work, Sociology, Criminal Justice, or a related discipline, along with a minimum of one year of experience in the mental health sector or a comparable social service environment. In the role of Behavioral Health Case Manager II, you will be tasked with delivering case management and rehabilitative support to individuals and families. Your responsibilities may include crisis intervention services and the coordination of wraparound or family teams for children. Key Responsibilities:
  • Deliver case management and/or rehabilitative support to individuals and families
  • Facilitate wraparound/family team coordination and meetings
  • Conduct assessments, develop care plans, and provide crisis interventions as necessary
  • Collaborate with various organizations to ensure clients access essential services
  • Offer immediate crisis intervention on a 24-hour basis
  • Assist in averting out-of-home or out-of-school placements
Required Qualifications:
Bachelor's degree in Psychology, Social Work, Sociology, Criminal Justice, or a related field
At least one year of professional experience in the mental health sector or a related social service field
Current certification as a Behavioral Health Case Manager II
