We are seeking a skilled Registered Nurse to join our Case Management team at Ascension St. John's Medical Center in Tulsa, OK. As a RN Case Manager, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.
Responsibilities- Coordinate interdisciplinary care plans for patients from admission to discharge
- Create and implement care plans that integrate patient and family preferences and values
- Monitor patient care through assessments, evaluations, and patient records
- Advocate for resources and removal of barriers to care
- Maintain ongoing dialogue with supervisors and healthcare providers to ensure effective implementation of care plans
- Act as a resource for medical record documentation, service appropriateness, and treatment options for post-discharge care
- Registered Nurse credentialed from the Oklahoma Board of Nursing required
- BLS Provider certification preferred
- Case Manager certification from the Commission for Case Manager Certification (CCMC) preferred
- Diploma from an accredited school of nursing or professional licensure at time of hire required
- Minimum 3 years of bedside nursing experience
- Case Management experience
