As a Case Manager with All Ways Caring HomeCare, you will play a vital role in ensuring the highest level of care and support for our clients. You will be responsible for receiving and responding to incoming calls from referral sources and potential clients, identifying their needs, and consulting with our Director of Clinical Management to determine our ability to meet those needs.
Key Responsibilities
- Complete all viable referrals by setting up and carrying out pre-screening assessments, completion of pre-screening reports, and preparation of case proposals and other related pre-admission paperwork.
- Oversee, direct, and supervise field staff assigned to assist with the pre-screening process.
- Access national, state, and company account information, including account names and terms of contracts or other past payer agreements.
- Consult with third-party representatives regarding client benefit coverage, client financial responsibility, company service authorization, and specific reimbursement procedures.
- Contact referral sources to advise them of case acceptance and provide information on the clinical team responsible for the client's case.
- Develop and maintain a working knowledge of all services and resources provided by the company and services available within the community.
- Maintain relationships with standard referral sources and payer case managers.
- Monitor and track referral sources' satisfaction levels, track and report on conversion ratios, and provide summary reports to management at requested intervals.
- Implement and maintain operational processes to ensure compliance with company policies, requirements, and regulatory mandates.
- Adhere to and participate in the company's mandatory HIPAA privacy program and Business Ethics and Compliance programs.
- Participate in quarterly growth planning meetings and activities, including discussions around staffing and recruitment needs.
- Bachelor's degree in a human services field or nursing field.
- Valid driver's license.
- Two years of experience in case management or a related discipline.
- Ability to communicate both verbally and in writing.
