We are seeking a highly skilled and compassionate Case Manager to join our team at Family & Children's Services. As a Case Manager, you will work with adults who have been diagnosed with a severe mental illness and/or co-occurring disorder, providing a range of services to support their recovery and well-being.
Key Responsibilities- Provide individualized support and services to clients, including income management, insurance navigation, housing assistance, and vocational or educational needs.
- Develop and implement crisis assessment and safety plans, as well as provide referrals to legal services as needed.
- Educate clients on various skills related to topics such as money management, symptom education, and interpersonal relationships.
- Collaborate with multidisciplinary teams to provide coordinated care and support to clients.
- Meet clients where they are in terms of current mental health symptoms and treatment preferences.
- Bachelor's Degree in a social sciences field, with consideration given for degrees in Criminal Justice or Education.
- Case Management II certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire.
- Valid Oklahoma Driver License and satisfactory driving record.
