**Job Summary:**

We are seeking an experienced LPN to join our team, providing one-on-one care to patients in need. This is a flexible scheduling opportunity, with various shifts available to accommodate your needs.

**Responsibilities:**

* Implement physician-prescribed care plans, ensuring the highest level of patient satisfaction.
* Conduct thorough patient assessments, identifying areas for improvement and developing targeted care strategies.
* Administer prescribed medications, treatments, and therapies as ordered by physicians.
* Document all patient interactions, updates, and progress in a timely and accurate manner.
* Educate family members on health promotion, disease prevention, and proper care techniques.
* Collaborate with Case Managers to coordinate patient care, resolve issues, and ensure seamless transitions.

**Requirements:**

* Active, unencumbered LPN license in the state of practice
* Current CPR certification from American Red Cross, American Heart Association, or BLS
* TB skin test (current within last 12 months)
* At least six months prior hands-on nursing experience preferred but not required
* Reliable transportation for travel to patient homes
