Job Summary:

We are seeking a highly skilled and dedicated Home Health Physical Therapist Assistant to join our team at All Medical Personnel. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality physical therapy services to patients in their homes.

Job Responsibilities:
  • Collaborate with the patient, family, and primary care provider to develop a comprehensive plan of care focused on achieving the patient's physical therapy goals.
  • Instruct, coach, and guide patients and families to achieve optimal health outcomes while promoting independence and quality of life.
  • Provide skilled, intermittent physical therapy in the patient's home, utilizing evidence-based practices for assessment, clinical decision-making, and coordination of care.
  • Act as case manager and team lead for patients requiring home health therapy services, when nursing is not involved or physical therapy is the primary patient need.
  • Assess patients and collaborate with the interdisciplinary team to develop a patient-centered plan of care.
  • Provide physical therapy treatment to patients and families as ordered by the physician.
  • Maintain accurate clinical documentation to support service delivery within specified timeframes.
  • Provide instruction to patients and caregivers and revise the treatment plan as necessary.
Requirements:
  • 1-2 years of experience in an outpatient rehabilitation setting.
  • At least two years of physical therapy experience preferred.
  • One year of experience in home health or another healthcare setting with a geriatric emphasis.
  • Current license in Physical Therapy or ability to obtain prior to hire.
  • American Heart Association Healthcare Provider BLS certification preferred upon hire and required within 90 days of hire.
  • Current Driver's license.
  • Excellent customer service skills.
  • Ability to work independently with minimal supervision.
  • Demonstrate professionalism when interacting with patients, families, physicians, and hospital staff members.
Compensation:

The estimated annual salary for this position is $60,000 - $80,000 based on location and experience.
