CCBHC Youth Acute Care-Emergency Department Liaison

Eskenazi Health is seeking a highly skilled CCBHC Youth Acute Care-Emergency Department Liaison to ensure continuity of care for Certified Community Behavioral Health Clinic (CCBHC) children and adolescent clients between the Sandra Eskenazi Mental Health Center CCBHC and acute care, emergency rooms, or the juvenile justice system as required.
Key Responsibilities- Transition of Care Coordination: Develop and maintain strong collaborations with acute care, emergency rooms, and juvenile justice system entities to ensure safe and healthy transitions of care for CCBHC clients.
- Clinical Coordination: Coordinate and help CCBHC clients navigate post-discharge transition to outpatient or other needed treatment to minimize time between discharge and treatment follow-up.
- Client Advocacy: Assist in advocating for CCBHC clients to ensure access to needed services, including developing community relationships and resources.
- Treatment Planning: Help the primary outpatient team coordinate and develop the client's treatment plan incorporating input from the client, client's family, and any representative with decision-making capacity.
- Crisis Prevention and Safety Planning: Develop, revise, or monitor crisis prevention and safety plans to reflect current CCBHC client status and need.
- Client Progress Monitoring: Monitor and document CCBHC client progress, including reasonable attempts to contact CCBHC clients post-discharge.
- Collaboration and Communication: Actively work with CCBHC outpatient treatment teams to coordinate transition of CCBHC client to community living post-acute care discharge, including attending relevant team staffings and participation in the Acute Services morning call.
- Education and Experience: Bachelor's degree in human services, psychology, social work, or a related field with two years of relevant work experience required; Master's degree in human services, psychology, social work, or a related field preferred.
- Licensure and Certification: Valid Indiana behavioral health and human services (BHHS) license in good standing that meets Indiana BHHS licensure requirements is required, dependent upon the degree conferred.
- Skills and Abilities: Basic computer skills, strong interpersonal skills, crisis intervention skills, motivational interviewing skills, ability to learn how to teach a skill based upon person-centered learning needs and learning styles, ability to be both clear and concise in written and verbal communications, ability to work cooperatively in a multidisciplinary team, ability to think critically and problem solve efficiently, ability to provide hands-on client care, and ability to perform safety interventions including physical safety interventions required for service area.
