Job Summary

The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, or assisted living facilities. As a contributing member of our interdisciplinary team, you'll provide expertise on psych-social assessments, interventions, and community resources. You'll work in either a Case Management, Admissions, or Resource role, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.

Responsibilities
  • Provide care and services to patients in their homes, skilled nursing facilities, or assisted living facilities
  • Contribute to the interdisciplinary team, providing expertise on psych-social assessments, interventions, and community resources
  • Work in either a Case Management, Admissions, or Resource role
  • Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Manage visits, documentation, meetings, and prioritize urgent needs
  • Learn and utilize an electronic medical records system
  • Travel across the service area with reliable transportation, valid driver's license, and auto insurance
Requirements
  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
  • Ability to work well within a team environment
  • Strong communication and time management skills
  • Ability to learn and utilize an electronic medical records system
