The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, or assisted living facilities. As a contributing member of our interdisciplinary team, you'll provide expertise on psych-social assessments, interventions, and community resources. You'll work in either a Case Management, Admissions, or Resource role, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Qualifications- Current RN Licensure - Hold a valid RN license in good standing in the state(s) of practice
- Teamwork and Collaboration - Work effectively within a team environment to achieve common goals
- Independence and Autonomy - Work independently, managing visits, documentation, meetings, and communication with healthcare professionals
- Time Management - Prioritize and manage multiple concerns, including urgent needs and scheduled visits
- Electronic Medical Records - Learn and utilize our electronic medical records system
- Reliable Transportation - Possess a valid driver's license, auto insurance, and reliable transportation to travel across the service area
