Job Summary

This role will work 5 days a week in 8-hour shifts, completing hospice admissions for new patients and assisting with visits throughout the week. Admissions occur in patient homes, including private residences, assisted living communities, and skilled nursing facilities. As a contributing member of the interdisciplinary team, you'll provide expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Requirements
  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
  • Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Strong communication skills to work closely with patients and the patient care team
  • Ability to work independently with activities such as phone, email, and written communication, meetings with healthcare professionals, and an understanding of hospice rules and regulations
  • Time management skills to manage visits, documentation, meetings, and urgent needs
  • Ability to learn an electronic medical records system
  • Reliable transportation and a valid driver's license
