Impact of the Role:

The TCL Clinician plays a crucial role in care coordination and management for individuals enrolled in the Transitions to Community Living Program (TCL). This position focuses on addressing the comprehensive needs of clients, including physical health, behavioral health, and social determinants of health. The clinician will implement care plans based on established national standards to ensure holistic support.

Key Responsibilities:

  • Conduct assessments to identify the mental health, medical, and social needs of clients.
  • Facilitate timely discharge and transition planning, ensuring clients with serious mental illness (SMI) can thrive in integrated settings.
  • Provide integrated care management and coordination for clients.
  • Evaluate the needs of clients transitioning into supportive housing.
  • Oversee the development and monitoring of individualized care plans, ensuring service delivery aligns with health and safety standards.
  • Collaborate effectively with the care team to enhance client outcomes.
  • Offer clinical consultation as needed.

Qualifications:

Education: Master’s degree required.

Experience: A minimum of one year of post-Master's experience in a clinical setting with mental health/substance use populations is necessary.

Preferred Experience: Experience working with individuals with severe and persistent mental illness in institutional settings is preferred.

Licensure: Must hold a valid license in North Carolina as a LCSW, LPA, LCMHC, LMFT, or LCAS. Dual licensure in mental health and substance abuse is preferred.

Additional Requirements:

Must possess a valid driver's license and reside within the designated region.
