As an RN MDS Coordinator, you will be instrumental in conducting precise MDS evaluations and formulating thorough care strategies for our patients. You will work closely with a multidisciplinary team to ensure exceptional patient care.

Key Responsibilities:
  • Conduct precise MDS evaluations
  • Facilitate patient care plan meetings
  • Ensure adherence to Medicare & Medicaid regulations
  • Engage in Quality Measure discussions
  • Identify and address documentation improvement areas
  • Support Medicare coverage determinations
Qualifications:
  • Current RN Licensure
  • Experience in Skilled Nursing / Long-Term Care MDS preferred
  • Proficient in Microsoft Office Suite
  • Excellent communication and organizational abilities
