American Senior Communities is seeking a Clinical Assessment Coordinator (RN). What will your responsibilities entail as a Clinical Assessment Coordinator? You will enhance personalized support and care for residents by overseeing the completion of evaluations utilizing RAI tools.

In this position, you will serve as an advocate for residents by managing the interdisciplinary care planning process.

Responsible for overseeing the management of resident assessments related to case mix reimbursement and ensuring MDS reports are submitted in accordance with established guidelines.

You will also participate as a member of Quality Improvement committees aimed at improving positive outcomes for residents.

Qualifications:
Exhibit C.A.R.E. values towards our residents, their families, customers, and fellow staff members.

Must be a graduate of an accredited School of Nursing, with a preference for a BSN. A minimum of one year in nursing management within the long-term care sector is required.

At least two years of professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing settings.

Familiarity with the Resident Assessment Instrument, the interdisciplinary care planning process, and Minimum Data Set 2.0 is essential. Knowledge of OBRA regulations and current state reimbursement guidelines is also necessary.

Understanding of current laws and regulations that affect the provision of care and services in nursing facilities is required.
