At Tandym Group, we are seeking a skilled Registered Nurse (RN) to join our team as a Hospice Referral Coordinator. This role is responsible for evaluating and establishing a plan of care for patients referred for home health services. The ideal candidate will have 2+ years of experience in Home Care Nursing and a strong background in Physical Assessment and Interviewing skills. Key responsibilities include:
Evaluating patient needs and developing a plan of care
Facilitating admission of referrals to home care programs
Developing and maintaining positive relationships with healthcare professionals
Performing other duties as needed
Qualifications:
2+ years of experience in Home Care Nursing
Graduate of an accredited School of Nursing
NYS Registered Nurse (RN) license
Proficiency in Physical Assessment and Interviewing skills
Solid assessment, clinical, and documentation skills
Great interpersonal and communication skills
Strong attention to detail and organizational skills are essential for this role.
