Resident Services Coordinator

We are seeking a highly skilled and compassionate Resident Services Coordinator to join our team at 2Life Communities. As a key member of our staff, you will play a vital role in providing exceptional support to our residents and their families.
Key Responsibilities- Conduct regular assessments of residents' social, psychological, cognitive, and physical status, and develop personalized plans to address their needs.
- Advocate on behalf of residents to ensure timely and cost-effective access to essential services.
- Participate in Housing and Health Care Integration Initiatives to promote seamless care coordination.
- Respond to resident emergency calls and provide support during scheduled work hours.
- Educate and assist residents and their families on available services and resources within 2Life Communities and the community.
- Pursue additional services through private, local, state, and federal sources to enhance resident care.
- Document and maintain confidential files of significant contacts with residents, social service providers, medical providers, families, and for HUD reporting.
- Coordinate care across medical providers for residents and ensure safe transitions in discharge planning.
- Masters or BA Degree in Social Work, Gerontology, or a related field.
- Resident Services Coordinator Certification or willingness to attain within the first year of employment.
- Experience working with older adults.
- General computer literacy with Microsoft Office products and database software.
- Bilingual in Russian (desired but not required).
