Job Summary

We're seeking a Community Health Worker to join our team at Sage Health. As a Community Health Worker, you'll play a critical role in delivering outreach education services, providing information on community resources, and encouraging patients to visit their Sage Health provider.

Key Responsibilities
  • Ensure patient information remains secure and confidential.
  • Develop a database of grant-funded and community resources to support patients.
  • Participate in center and market interdisciplinary team meetings to discuss patient care and outcomes.
  • Assist patients in applying for government and community assistance programs.
  • Provide concise and thorough documentation of patient progress and referrals.
  • Support patients in improving their well-being by staying out of the hospital and attending regular visits to their primary physician.
  • Act as a liaison and advocate between patients, families, physicians, and facilities/agencies.
  • Proactively engage patients to manage their own health and healthcare decisions.
  • Provide patients with education on community resources and benefits.
Requirements
  • Currently resides in the Sage Health Medical Center City/County.
  • Valid driver's license, car insurance, and access to an automobile.
  • Ability to travel between multiple healthcare facilities and patient homes.
  • At least two years of experience working with vulnerable populations.
  • Excellent interpersonal, written, and verbal communication skills.
