Located in Chicago, Illinois, the Rush Cancer Center is seeking a skilled professional to fill the role of Oncology Nurse Navigator 2. This full-time position involves working with physician practices and multidisciplinary teams to ensure effective service utilization and care coordination for patients within the Cancer Center's service line.

The estimated salary for this position is $95,000 - $110,000 per year, depending on experience.

About the Role:
This Oncology Nurse Navigator 2 will be responsible for assessing patients' psychosocial, financial, and clinical needs throughout their care continuum, ensuring high-quality care while supporting quality metrics for the Cancer Center and Rush University Medical Center. The successful candidate will work as a liaison between patients, physician practices, the hospital, and community organizations, performing functions that meet core competencies of oncology navigation, including care coordination, communication, education, and professional role/care. Additionally, they will contribute to the assessment of patients' needs, provide symptom management using evidence-based practice, and perform various administrative tasks related to patient care.

Responsibilities:
- Establish a caring therapeutic relationship with patients and families, utilizing effective communication while managing a defined patient caseload.
- Assess patients for social determinants of health and develop a plan of care and mutual goal setting.
- Serve as a liaison for the patient, becoming an expert on the patient and their needs throughout the trajectory of their care from diagnosis to end of life or survivorship.
- Advocate for patients and their families to ensure timely access to treatment and equal access to healthcare.
- Collaborate with all members of the healthcare team in a positive, professional manner to ensure coordination of patient care.

Requirements:
- Current State of Illinois Registered Nurse licensure required.
- At least one year of nursing experience.
- Earned nursing degree of baccalaureate level of education or higher.
- Oncology nursing experience preferred.
- Oncology nursing certification preferred.
