We are seeking a highly skilled and compassionate Social Services Case Manager to join our interdisciplinary treatment team at Peachford Hospital. As a key member of our team, you will be responsible for providing quality therapeutic and case management services to our patients from admission to discharge.

Responsibilities
  • Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus.
  • Provide individual, group, and family solutions-focused interventions.
  • Facilitate process and didactic groups.
  • Develop an individualized discharge plan utilizing information obtained from the patient, treatment team, and family to determine patient needs.
  • Identify family or community resources such as group homes, halfway houses, or treatment providers.
Requirements

A license-eligible master's degree in a related clinical area is required, and previous inpatient/outpatient hospital experience is preferred. Licensure is also preferred.

Qualifications
