Job Description

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with an interdisciplinary team to deliver whole-person care.

Responsibilities
  • Provide direct patient care, including assessments, medication management, and wound care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
  • Communicate effectively with patients, families, and caregivers to ensure understanding and satisfaction
  • Participate in ongoing education and training to stay current with best practices and industry developments
Requirements
  • Associate's degree in nursing
  • Current Washington Registered Nurse License
  • National Provider BLS - American Heart Association certification
  • National Provider Identifier (NPI) number
Preferred Qualifications
  • Bachelor's degree in nursing
  • Three to Five (3-5) Years in Acute care or community behavioral health setting
  • Community behavioral health and geriatric experience
