Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and caregivers, providing education and support to promote healthy outcomes.
- Document patient progress, identifying areas for improvement and implementing changes as needed.
- Associate's degree in Nursing or higher.
- Current Washington State Registered Nurse License.
- National Provider BLS certification within 30 days of hire.
- Ability to obtain an NPI number within 30 days of starting in the position.
