About this Opportunity

We are seeking a skilled Case Management Specialist to join our team at The Salvation Army. As a key member of our life management program, you will work closely with clients to develop comprehensive, long-term plans for recovery and provide ongoing support and guidance.

Key Responsibilities
  • Establish and implement individualized program plans for clients, taking into account their unique needs and circumstances.
  • Provide counseling and mentoring services to clients, helping them to modify or stop negative behaviors and achieve their goals.
  • Collaborate with external case management professionals to ensure a holistic approach to client care.
  • Prepare and process vouchers for clients, directing them to resources such as the Family Store or Food Pantry as needed.
  • Plan and coordinate life management classes and workshops to help clients develop skills and strategies for coping with their environment.
Requirements
  • Bachelor's degree in Social Work, Behavioral Science, or a related field.
  • Three years of progressively responsible experience in case management, with a focus on direct social services and individualized planning.
  • Valid State Driver's License (preferred).
Working Conditions

This role requires the ability to work in a shelter environment, which may involve exposure to physical health risks and unsanitary conditions. You will also be required to drive a vehicle to off-site meetings and home visits, which may involve physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
