Job Title: Service Coordinator

About the Role:

The Service Coordinator will provide case management services to individuals and families, ensuring that they receive the necessary support and resources to achieve their goals. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities:
  • Provide case management services to individuals and families, including assessment, planning, and coordination of services.
  • Develop and maintain relationships with community agencies and organizations to ensure access to resources and services.
  • Conduct home visits and other site visits to assess client needs and provide support.
  • Collaborate with multidisciplinary teams to ensure comprehensive service delivery.
  • Maintain accurate and timely records of client interactions and service delivery.
Requirements:
  • Bachelor's degree in a related field, such as early childhood education, psychology, or social work.
  • At least 3 years of experience in case management or a related field.
  • Valid Pennsylvania driver's license and reliable transportation.
  • Ability to work in an open office environment and communicate effectively through virtual technology.
