Skilled LVN Care Coordinator

Job Summary:
The Skilled LVN Care Coordinator provides direct nursing care to residents, supervising patient care performed by certified nursing assistants (CNAs). This role ensures care is in accordance with federal, state, and local standards, as well as facility guidelines and regulations.
Duties & Responsibilities:
This position involves providing individualized nursing care using a systematic approach. It requires establishing priorities based on essential needs and available resources of time, personnel, equipment, and supplies.
- Coordinate resident admissions, discharges, and transfers
- Makes regular rounds to assess physical and emotional status, initiating necessary nursing interventions
- Performs nursing evaluations regarding the health status of the resident, accurately documenting symptoms, responses, and status
- Collects data for assessments (MDS/RAPs) and develops plans of care, entering and recording data accurately and in a timely manner
Nursing Interventions:
The Skilled LVN Care Coordinator develops and implements plans of care based on assessments. This includes selecting and instituting appropriate nursing interventions to stabilize a resident's condition and prevent complications.
Collaboration and Communication:
This role involves consulting with physicians and licensed independent practitioners, clarifying orders or treatment regimens when necessary. The coordinator must correctly administer medications and treatments, adhering to facility policies.
Education and Counseling:
The Skilled LVN Care Coordinator designs, promotes, and organizes resident education and counseling based on health needs. This includes collaborating with the resident, healthcare team members, and significant others to provide continuity of care.
Nursing Rehabilitation and Restorative Nursing:
This position involves planning, organizing, managing, and coordinating nursing rehabilitation and restorative nursing care. The coordinator initiates nursing interventions promoting residents' ability to adapt and adjust to living independently.
Supervision and Teaching:
The Skilled LVN Care Coordinator supervises nursing staff performing nursing rehabilitation and restorative activities. They teach nursing assistants about prevention measures and proper use of these interventions.
Salary: Estimated $55,000 - $65,000 per year
