Skilled Home Health Nurse Professional

We are seeking a skilled and dedicated Licensed Vocational Nurse to join our team at Sutter Health.
About the Role
The successful candidate will provide high-quality patient care, working under the general direction of a qualified registered nurse. Key responsibilities include instructing patients and caregivers in self-care, providing basic nursing care as needed, and maintaining accurate records of patient treatment plans.
Main Responsibilities:
- Deliver exceptional patient care by instructing patients and caregivers in self-care techniques and providing necessary support.
- Assist with maintaining detailed records of patient treatment plans, ensuring accuracy and thoroughness.
- Cultivate effective communication with patients, families, and healthcare teams to guarantee quality care and services.
- Provide emotional support and guidance to patients and their families during challenging times.
- Engage in ongoing education and professional development to stay current with best practices and industry advancements.
Requirements and Qualifications
To be considered for this role, candidates must possess the following qualifications:
- Equivalent experience accepted in lieu of required degree or diploma.
- Graduate of an accredited school of Vocational Nursing or equivalent education/experience.
- LVN-Licensed Vocational Nurse certification.
- BLS-Basic Life Support Healthcare Provider certification.
- CPTI-Certified Phlebotomy Technician I within 30 days of hire.
- Valid California Driver's License Class C.
- Automobile Insurance.
PREFERRED EXPERIENCE AND SKILLS
- Minimum one year of professional experience required.
- Home health or community health experience preferred.
- Five years of direct patient care experience within the past five years required.

- Basic competencies in the fundamentals of nursing and pharmacology.
- Demonstrated skills in effective written and verbal communication.
- Ability to recognize the needs and concerns of diverse groups of people.
- Ability to maintain harmonious and constructive working relationships with internal and external customers.
- Ability to use and give instruction on adaptive equipment.
- Maintain a positive and professional image, demonstrating self-directed learning to meet professional development requirements.
- Responsible for maintaining all required licensure and certifications.
Work Schedule and Salary
This is a full-time position with variable hours, Monday through Friday. The estimated salary range is $34.90 - $46.87 per hour. This role may require regular work with hazardous materials.
