Extended Care Sober Living Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at Mountainside Treatment Center. As a Case Manager, you will play a vital role in providing guidance and support to clients in our Extended Care program.
Key Responsibilities- Provide follow-up and support to clients in Extended Care, ensuring they receive the necessary guidance and resources to reintegrate into their daily lives.
- Communicate effectively and timely with designated clients, families, and colleagues to ensure seamless coordination of care.
- Address non-therapeutic needs of clients while maintaining a professional demeanor, ensuring a high standard of service.
- Prepare for new admissions, ensuring all materials are in place, and complete the admissions process, including greeting new clients and their families.
- Orient new clients to the facility, programs, and services, providing a comprehensive understanding of their treatment plan.
- Communicate behavioral problems or safety concerns to Managers and Counselors, ensuring prompt attention and resolution.
- Perform routine inspections of client rooms, reporting infractions of resident guidelines, and facilitate program groups and other activities as needed.
- Aid team members to ensure clients receive an unsurpassed level of service, and manage scheduling for extended care clients.
- Provide real-time mentorship and guidance to residents, assisting with arranging transportation for residents to meetings and appointments.
- Ensure residents attend the required number of recovery support meetings, and contact clients' emergency contacts, family members, and former providers as needed.
- Formulate and implement recovery plans for residents, coaching clients individually and in groups to overcome alcohol and drug dependency.
- Provide weekly updates to primary contacts and referents, and refer residents to other support services as needed.
- Monitor clients' progress and adapt treatment plans as necessary, and assist with coordinating treatment for clients with Outpatient Services.
- Prepare and maintain reports and case histories, ensuring proper documentation in EMR.
- High School Graduate or equivalent in GED
- Current or working towards certification (CAC, CASAC, CPRS)
- Minimum 1 year's experience/knowledge in substance abuse treatment preferred
- Clean Driving Record
The base rate of pay for this position is $16.00 to $24.00 per hour, with actual pay determined based on a number of job-related factors. We offer a comprehensive benefit package, including paid time off, paid holidays, 401(k) with employer matching, and free meals while working on the Canaan campus.
Our Wellness Reimbursement benefit is meant to encourage employees to engage in productive self-care to avoid burnout and compassion fatigue, with a monthly reimbursement of $75.00.
