Job Summary

The Behavioral Health Associate works in conjunction with the outpatient treatment team to develop, coordinate, and disseminate continuing care plans with patients. This role requires excellent communication and interpersonal skills, as well as the ability to provide exceptional service to internal and external customers.

Key Responsibilities
  • Meet with patients to research discharge planning needs as presented by the treatment team.
  • Meet one-on-one with patients from admission until discharge at a minimum of one time per week to discuss case management.
  • May be assigned to conduct educational lectures or group activities.
  • Communicate with the Business Development Department and referring providers to coordinate appropriate aftercare resources.
  • Under the guidance of the Clinical Director, review and research aftercare resources.
  • Support the treatment team and patients in communication with referral and continuing care resources.
  • Document continuing care plans, communications, and meetings.
  • Prepare and distribute copies of patient schedules weekly as requested.
  • Attend treatment team meetings as assigned.
Requirements
  • Bachelor's Degree in Behavioral Science or related field preferred.
  • One year's experience working in a behavioral healthcare facility preferred.
  • Ability to provide excellent service to internal and external customers.
