Our team is dedicated to providing evidence-based care for individuals struggling with drug and alcohol addiction, trauma, sexual addiction, behavioral health conditions, and co-occurring disorders.

Job Summary

The Behavioral Health Associate works closely with the outpatient treatment team to develop, coordinate, and disseminate continuing care plans with patients. This role requires strong communication and organizational skills, as well as the ability to provide excellent service to internal and external customers.

Key Responsibilities
  • Meet with patients to research discharge planning needs and develop continuing care plans.
  • Meet one-on-one with patients from admission until discharge to discuss case management and provide support.
  • Conduct educational lectures or group activities as assigned.
  • Communicate with the Business Development Department and referring providers to coordinate aftercare resources.
  • Review and research aftercare resources under the guidance of the Clinical Director.
  • Support the treatment team and patients in communication with referral and continuing care resources.
  • Document continuing care plans, communications, and meetings.
  • Prepare and distribute patient schedules as requested.
Requirements
  • Bachelor's Degree in Behavioral Science or related field preferred.
  • One year's experience working in a behavioral health care facility preferred.
  • Ability to provide excellent service to internal and external customers.
