Job Description

At At Ease Home Health Care Inc, we are seeking a skilled and compassionate Registered Field Nurse to join our team. As a Home Health Care Nurse, you will play a vital role in providing high-quality in-home health care to patients in Los Angeles County, Ventura County, and Orange County.

Key Responsibilities:
  • Patient Care:
    • Conduct thorough assessments of patients and their families to determine home care needs.
    • Develop and implement personalized care plans that establish goals based on nursing diagnosis.
    • Administer medications and treatments as prescribed by physicians.
    • Counsel patients and their families on meeting nursing and related needs.
  • Communication:
    • Prepare and update clinical notes and communicate with physicians as necessary.
    • Coordinate care plans with community health-related persons.
  • Additional Duties:
    • Participate in on-call duties as defined by the on-call policy.
    • Ensure arrangements for equipment and other necessary items and services are available.
    • Instruct, supervise, and evaluate home health aide care provided.
Requirements:
  • Active RN license.
  • One year of nursing experience (training and oversight may be an option for new graduates).
  • Current physical, including negative TB test or X-ray.
  • Current CPR certification.
  • Covid Vaccine (as per CDPH requirements).
  • Ability to walk, bend, stoop, and lift objects weighing up to 25 lbs.
  • Active legal resident status.
  • Licensed driver with an automobile that is insured in accordance with state and/or organizational requirements.
