Job Summary

We are seeking a highly skilled and compassionate Home Health Nurse to join our team at KA Recruiting Inc. as a Skilled Home Health Care Professional. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to patients in their place of residence.

Key Responsibilities:

  • Assess patients for skilled home health needs and develop a plan of care with physician approval.
  • Provide nursing care ordered by the physician to patients in their place of residence, continually re-evaluating the patient's nursing needs.
  • Perform baseline-nursing skills, including assessment, wound care, catheter care, and infusion, maintaining clinical competencies according to agency standards.
  • Complete comprehensive assessments, including OASIS items, accurately and timely at specific time-points as defined by regulation and agency policy.
  • Develop a plan of care, in collaboration with the patient, family/caregiver, and physician orders, including frequency and duration of nursing visits, specific care or treatments to be provided, appropriate goals, medications, other disciplines needed, and discharge plan.
  • Implement and provide nursing care, treatment, and services according to the plan of care and accepted standards of care, including patient education and any IV or other medications required to be administered by the nurse.
  • Communicate changes in patient status, patient issues, and concerns, and changes in orders to the Physician, Clinical Manager, and Clinical Supervisor according to agency guidelines.

Requirements:

  • Current licensure as a Registered Nurse (RN) in the state of practice.
  • Experience in home health care or a related field.
