Job Title: Home Health Field Nurse

We are seeking a skilled and compassionate Home Health Field Nurse to join our team at At Ease Home Health Care Inc. As a Home Health Field Nurse, you will provide high-quality in-home health care to patients in Los Angeles County, Ventura County, and Orange County.

Job Summary:

The Home Health Field Nurse will be responsible for developing and implementing individualized care plans for patients, conducting thorough assessments, and providing education to patients and their families. The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for delivering exceptional patient care.

Responsibilities:
  1. Conduct thorough assessments of patients to identify their needs and develop individualized care plans.
  2. Provide high-quality in-home health care to patients, including wound care, medication management, and education on disease management.
  3. Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  4. Document patient progress and outcomes in a timely and accurate manner.
  5. Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Active LVN/LPN license
  • One year of nursing experience (new grads may be considered with training and oversight)
  • Current physical, including negative TB test or X-ray
  • Current CPR certification
  • Covid Vaccine (as per CDPH requirements)
  • Ability to walk, bend, stoop, and lift objects weighing up to 25 lbs.
  • Active legal resident status
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
