Job Summary

We are seeking a highly skilled Service Coordinator to join our team at Elevance Health. As a Service Coordinator, you will play a critical role in providing support to individuals with long-term services and supports (LTSS) needs.

Key Responsibilities
  • Conduct face-to-face assessments and develop comprehensive Person-Centered Support Plans (PCSPs) for individuals with LTSS needs.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and management of individual's physical health, behavioral health, and LTSS needs.
  • Identify and address gaps in care, and coordinate with clinical healthcare management to provide cost-effective and efficient utilization of LTSS.
  • Document short and long-term service and support goals in collaboration with the individual's chosen care team.
  • Identify opportunities for alternative levels of service or other waiver programs.
  • May serve as mentor, subject matter expert, or preceptor for new staff, and participate in process improvement initiatives.
Requirements
  • Requires a BA/BS degree and a minimum of 2 years of experience working with a social work agency, or any combination of education and experience that would provide an equivalent background.
  • Specific education, years, and type of experience may be required based on state law and contract requirements.
Preferred Skills and Experiences
  • BA/BS degree in a healthcare-related field is preferred.
