Job Summary

We are seeking a highly skilled LTSS Service Coordinator to join our team at Elevance Health. As a Service Coordinator, you will play a critical role in conducting service coordination functions for a defined caseload of individuals in specialized programs.

Key Responsibilities
  • Conduct face-to-face program assessments to identify individual needs and develop comprehensive Person-Centered Support Plans (PCSPs).
  • Collaborate with individuals, families, and caregivers to facilitate the Person-Centered Planning process and ensure that individual preferences, needs, and goals are met.
  • Interface with Medical Directors and participate in interdisciplinary care rounds to support the development of fully integrated care plans.
  • Engage individuals' circles of support and manage their physical health, behavioral health, and LTSS needs, as required by applicable state law and contract, and federal requirements.
  • Perform face-to-face program assessments using various tools with pre-defined questions and apply motivational interviewing techniques for evaluations, coordination, and management of individual waiver needs.
  • Identify members with potential clinical healthcare needs and coordinate their cases with the clinical healthcare management and interdisciplinary team to provide care coordination support.
  • Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of long-term services and supports.
  • Document short and long-term service and support goals in collaboration with individuals' chosen care teams.
  • Identify members who would benefit from alternative levels of service or other waiver programs.
  • May serve as a mentor, subject matter expert, or preceptor for new staff and participate in process improvement initiatives.
Requirements
  • Requires a BA/BS degree and a minimum of 2 years of experience working with a social work agency or any combination of education and experience that would provide an equivalent background.
  • Specific education, years, and type of experience may be required based on state law and contract requirements.
Preferred Skills and Capabilities
  • BA/BS degree in a healthcare-related field is preferred.
