We are seeking a highly skilled and compassionate LTSS Service Coordinator to join our team at Elevance Health. As a key member of our care coordination team, you will play a vital role in ensuring that individuals with complex needs receive the support and services they require to live independently.
Key Responsibilities- Conduct face-to-face assessments with individuals to identify their needs and develop personalized care plans.
- Collaborate with interdisciplinary teams, including medical directors, to ensure comprehensive care coordination and support.
- Engage with individuals, their families, and caregivers to understand their preferences, needs, and goals.
- Develop and implement Person-Centered Support Plans (PCSPs) that address physical health, behavioral health, and long-term services and supports needs.
- Coordinate with healthcare providers, service providers, and other stakeholders to ensure seamless care transitions and effective service delivery.
- Identify opportunities for cost-effective and efficient utilization of long-term services and supports.
- Document and report critical incidents, appeals, and grievances in accordance with regulatory requirements.
- Bachelor's degree in a healthcare-related field or equivalent experience.
- Minimum of 2 years of experience working in a social work agency or similar setting.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to travel to worksites and other locations as needed.
