As a Hospice/Home Health CNA, your key responsibilities will include:

  • Delivering services as outlined in each patient's hospice care plan.
  • Assisting with daily living activities such as bathing, grooming, light housekeeping, meal preparation, shopping, transportation, and companionship.
  • Supporting mobility, transferring, and conducting range of motion exercises.
  • Providing medication reminders, documenting patient conditions, and alerting supervisors to any concerns.
  • Ensuring a safe home environment with clear pathways.
  • Engaging clients in enjoyable activities like puzzles, games, reading, and hobbies.

Essential qualifications for the Hospice/Home Health CNA role include:

  • A high school diploma (or equivalent) and active CNA Certification in the relevant state.
  • A minimum of 1 year of experience as a CNA.
  • Prior experience in hospice care.
  • Current CPR certification.
  • A valid driver's license, auto insurance, and reliable transportation.
  • A compassionate demeanor, effective communication skills, and the ability to lift up to 50 lbs.
